Improving the dietetic service provided at the mental health unit

Problem:

– appropriate dietetic referral

– recognition of malnutrition

– recognition of refeeding syndrome

– correct prescription of oral nutritional supplements

– safe discharge Communication between catering and dietitians

– weight management waiting list

 

Actions:

– provided and sponsored new referral system

– introducing MUST on the ward and give education

– providing education regarding refeeding syndrome

– dietetic involvement on ONS prescription, dietetic assessment, provided guidance on minimal wards stock

– including dietetic assessment to the discharge summary and give ONS on discharge

– establish new system of communication between catering and dietitians

– providing healthy eating/weight management group sessions for weight management patients
